RECRUITMENT OF USERS UNDER THE "PATRONAGE CARE +" PROJECT
Dimitrovgrad Municipality, in its capacity of beneficiary under project BG05M9OP001-6.002-0104-C01 "Patronage care + in Dimitrovgrad municipality" , Operational Program "Human Resources Development" 2014-2020 ,
Announces:
We have started accepting documents from candidates for consumers for provision of hourly integrated health and social services in the home environment - home care since 17.05.2021.
Acceptance of documents for candidate users is open . The users who can use the services are from the following target groups:
1. persons over 65 years of age unable to self-service;
2. persons with a valid expert decision of a labor-expert medical commission;
3. persons with chronic disease (s), in need of long-term monitoring and health care;
4. persons after operative intervention and after hospitalization, in need of short-term observation and health care;
5. persons quarantined in connection with COVID-19;
6. persons over 54 years of age;
Required application documents:
1. Application form;
2. identity document (for reference);
3. identity document of the legal representative ;
4. expert decision ;
5. epicrisis from last treatment in a hospital institution ;
6. other current medical documents;
7. certificate for guardianship;
8. personal doctor of the user ;
9. current address .
Each person from the target group can receive health and social services (patronage care) up to 2 hours a day, according to the prepared preliminary needs assessment and subsequent individual needs assessment, individual support plan and the concluded contract for use of the service.
Applications can be obtained at 35 Zahari Zograf Street, first floor, south entrance , every working day from 08:30 to 17:00.
